Description/Eligibility/Service Details

Description	Focus of this team is on consultation, recommendations, and diagnosis. Workers will go into nursing homes; retirement homes and private residences to meet with individuals. Clinic appointments are also available. Psychiatric symptoms in elderly and age-related psycho geriatric symptoms are assessed and diagnosed with recommendations and appropriate referrals made.

Application	Referral for assessment may be initiated by the family (primary) doctor concerned that a senior may be suffering from a psychological, behavioural or memory problem For assessment, a primary physician's active involvement is required
