Facilitates access and/or develop community plans for transitional aged youth and adults with complex mental health and /or addictions challenges to care that is coordinated, uninterrupted, developmentally appropriate, comprehensive and flexibly adapted to the needs of the client Represents coordination efforts for persons with complex needs from the following Child, Youth, and Adult Social Service sectors: Acquired Brain Injury, Addictions & Mental Health, Criminal Justice, Developmental Disabilities, and Dual Diagnosis |
